Apply Online or Call for Certification Appointment:

Apply for WIC online or call your County or Tribal WIC agency to schedule a certification appointment. Once an appointment is set, you must go in person and bring required documents and all children up to age five to complete your enrollment.

The WIC appointment will take about 30 minutes and will include a nutrition/physical assessment.

What you will need to bring to your appointment to apply:

• Picture identification (ID) such as a birth certificate, driver’s license, passport, photo ID, social security card, etc.
• Proof of address such as a current driver’s license, a bill, a rent receipt, or personal mail with your name and address on it.
• The person(s) applying for WIC benefits.
• Proof of income such as work pay stubs, a recent tax form, a child support or unemployment check, etc.

• Citizenship Requirements: None
• Residency Requirements:  You must be a Minnesota resident
• Other:  If you or a family member is currently participating in any of the following programs, you and your family are income eligible for the WIC program:

            1.  Medical Assistance
            2.  MinnesotaCare
            3.  Food Stamps
            4.  Minnesota Family Investment Program (MFIP)
            5.  Energy Assistance
            6.  Free or Reduced Priced School Meal
            7.  Head Start

Or, if you or a family member is currently receiving Supplemental Security Income (SSI) or Medical Assistance - TEFRA, the person receiving these benefits is income eligible for the WIC Program.
